Please fill out the below to the best of your ability. Captain and angler information is required and used for all events including captain's dinner entry, awards dinner entry, auction, donations, etc.

Important Notice for Tournament Participants - Please be aware of the following policies regarding event tickets
-Captain's Dinner (8/14/25): Each registered team will receive 2 tickets to this event​
-Awards Dinner (8/15/25): Each registered team will receive 4 tickets to this event
No Exceptions. Walk-ins will not be permitted due to very limited space. Only those listed on this form will be admitted. Thank you for your understanding

APPAREL SIZING INFORMATION
Each registered team will receive 1 captain's bag including tournament apparel. There will be 2 sweatshirts and 4 t-shirts per bag. Please provide sizing for your team below. If sizing is not provided by 6/1/25 your bag will include 1 LG/ 1 XL sweatshirt and 1 MD/ 2 LG/ 1 XL t-shirts.​
GENERAL ENTRY FEE:
Entry $2,500.00

**Note: Cash and Checks will be accepted at North Chatham Outfitters both in person or by mail. Payments must be made by April 15th, 2025 and will not be accepted at the Captain's Meeting. If entry payment is not received by 4/15/25 your boat will be removed from the fleet, allowing entry to waitlisted boats.

Terms of Agreement: The Monomoy Shootout (tournament) is to be fished at the discretion of Captain and Crew. Any harm, loss, damage incurred during the events of the Monomoy Shootout are the sole responsibility of each team's Captain and Crew. Any material (photos, videos, etc) captured by the tournament committee during the event are property of the Monomoy Shootout and can be used on the tournament website and for promotional purposes. By signing the entry form all anglers acknowledge they have read and will abide by the tournament rules (www.themonomoyshootout.com). Those associated with the tournament assume no responsibility/liability for boats entered or individuals participating in the tournament. By signing, you agree to indemnify and hold harmless for all claims and costs, including attorney fees, arising out of your conduct/participation in The Monomoy Shootout.
